  • Your Size and Measurements (please see our measuring guide).
    Please understand that the size that actually needs to be ordered will be in accordance with the specific
    manufacturer's own size/measurement chart, and alterations may still be required for that perfect fit.
  • Date that design is needed to be worn
  • Your name
  • Telephone No.
  • Shipping address
  • Billing Address (if different)
  • Credit card information for required deposit

      It would be best to have alternate selections ready, in case your first selection is not available in time for your wear date.  

     Your credit card will never be charged until your order is confirmed.
